Instructions for Revision List Template:
Please review the general transcription rules at

especially sections I.1, I.2, II and IV.
* Page # -
The page number in the original document on which this record appears.
If there is no apparent page number, can be left blank.
If there are Page #s of left and right side of the image, take from the RIGHT side (only #).
For the whole image there are going to be ONE Page # for all families on the images.
* Registration # -
The Family Registration Number for this family during this census count.
This field MUST be filled in, in order to group family members together.
If there is no apparent Family Registration Number in the original document,
then create an artificial number for this family (That will be done only by Second reader! First reader, please keep the field empty.)
Use the same number for every member of the family.
For Supplemental Revision Lists, use a reference such as "2 to 43" to mean that this
registration Ò2Ó on the additional revision list is related to registration number 43 on the
primary revision list.
In the images the Registration # might be in a column "Family #" and under one number
there going to be a second number which is for the Former Registration # or if this is 10th Revision,
might a column -10th revision and another column 9th revisionÉ 10th - is the current Registration #
and 9th - Former Registration #, etc.
* Surname -
Family Name (hereditary surname) of this individual.
If there is no apparent surname, which is the case in many early revision lists,
then just put a hyphen ("-") in this field.
All surnames should be written in all CAPITAL LETTERS.
Other names (given names, town names) should have
an initial capital letter only.

If the surname, name, etc. are unclear, please put "?" in the cell. 
You can suggest a name and add a "?".  The "Second" reader should try to resolve these "??" 
If the second reader is still not clear, you could put into a comment column: surname is illegible, first name is unclear, etc. 
put always your own comment in brackets [ . . . ], like [Surname is illegible] or [Possible a mistakeÉ], etc.
* Given Name -
The Given Name(s) of this individual.
* Father -
The Given Name(s) of the Father of this individual.
This is often given in the form of a patronymic (i.e. a name ending with "-ovich" or "-ovna").
For example, if the record says "Khaimovich" or "Khaimov", write only "Khaim" in this field.
* Relationship to Head of Household -
This field describes how each individual in the household is related to
the head of the household.
For example: "Head of household", "Wife", "Son", "Daughter", "Daughter-in-law", "Grandson", "Brother", "Nephew", etc.
Start the work with upper case letter.
* Sex -
Enter "M" for Male, or "F" for Female.
This data is not explicitly present in the original Revision Lists,
but can be inferred based upon which side of the page the individual is on.
Males were listed on the left side of the page, with females on the right side.
The Revision Lists for certain years contained only males.
* Age -
Age of the individual, given in the present census enumeration.
Most often expressed in years.
Leave this field blank if the individual is deceased since the last Revision.
* Age at Last Revision -
Age of the individual, in the previous census enumeration.
Leave this field blank if the individual was not yet born at the time of the prevision revision.
* Reason Left -
This field explains what happened to individuals who were registered on the
previous revision list, who are not in the present revision.
People left for a variety of reasons. Explanations can include:
"Moved to . . .", "Died", "Expelled from the community", "Converted", "Divorced", "Married"
* Year Left -
The year in which the individual moved, died, etc.
* Comments -
Any comments about the individual which do not fit into any other field.
For example, the clarification of the relationship.
Any comment by the translator which does not appear in the original record
should be recorded in square brackets [ . . . ].
For a child if there is year she/he was born - put it in the comment column: Born in 1856
If a person is a Merchant of 3rd Gild - put this in the Comment, or a farmer - also in the comment.
If you think that there is a mistake in the records, or record is illegible, please put in brackets as [Illegible].
* Former Registration # -
The Family Registration Number for this individual during the previous census count.
If there are several prior registration #s, put 1835-#30; 1848-#128
If there is only one prior registraton - put only number, like 30
In the images the Registration # might be in a column "Family #" and under one number

* Day / Month / Year -
The date that this revision was taken.
* Town / Uyezd / Gubernia -
The location that this revision was taken: the Town / District / Province,
as was recorded on the top or title page of the original record.
* Type of Record -
Denotes if the record is a "Revision List", or an "Additional Revision List" (a list made
after the original revision, documenting those who may have been absent at last revision),
or any other type of list.
* Archive/Fond -
Leave it blank
* Microfilm # -
The microfilm number from the LDS Family History Library,
a seven-digit number.
If the records have not been microfilmed, write "Not microfilmed" in this cell.
